Well, sometimes it does. Sometimes it doesn't.
In fact, this second usage -- meaning "almost completely" -- confused me for years. I just avoided the usage because it didn't make sense. I totally agree it ought to go away.
Autistic Avenue hates these sorts of common phrases that don't make literal sense. They all but drive me mad.
