Why is he "the little" Saint Nick? He's big and fat, he's not little. Is there a different Saint Nick who is, in fact, little? Is this some slang term I'm unfamiliar with?

Probably as a term of endearment.
They say 'Little' Saint Nick because the song is supposed to be viewed as being sung by a personification of the thorough commercialization of the holiday season. Nick is referred to as 'little' because it's a shorthand for the condescending, patronizing, often sarcastic tone with which Santa is portrayed in contemporary media; Nick being referred to as 'little' is a metaphor for how 'little' we care for the true spirit of the holiday season; referring to nick that way shows how dismissive we are of it.
